    Sponsor Registration - Derby Day 2020
    ​Sponsorships will be granted on a first come first serve basis by date payment is received. Deadline for registration and payment is April 15 of the year of the event. Payments may be made via check mailed to HCCS PO Box 3417 McDonough, GA 30253 or through the "Donate" page at www.henrycountycancerservices.com. Payments must be received by 4/20/2017. I confirm I am the responsible party for this payment and that I am authorized by the company listed above to enter into a sponsorship agreement. By sponsoring this event and submitting this form, I authorize Henry County Cancer Services to utilize the company name listed above and any logo provided in the file upload feature below on marketing material related to this event.
